New patients are asked to arrive 30 minutes early to complete registration. You can save that time by completing the forms at home — see Patient Forms.
Please bring your photo ID, all insurance cards, your co-payment, and every medication you take in its original container. If the patient is a minor, bring the child's and the parent's Social Security numbers.

Common questions
Is a request the same as a booked appointment?
No. A request tells us you would like to be seen and when suits you; the office will call you back to agree a definite time. If you need to be seen today, please call (951) 303-6158 rather than using this form.
How quickly will I hear back?
The office responds during business hours — Monday to Thursday 9:00am to 5:00pm and Friday 9:00am to 12:00pm. Requests sent overnight or at weekends are picked up the next working day.
What if I need to be seen today?
Call the office early in the day on (951) 303-6158. Same-day slots are held open for people who are unwell.
